WCYC will provide the basic foundation for the fundamentals of cheerleading. The purpose of
this program is to encourage the highest sportsmanship while providing a cheer squad for the youth football program (WCYF). We aim to provide a fun, learning environment that gives all participants the opportunity to learn the skills necessary to support one another. We appreciate your full cooperation and support.

I, the undersigned parent/guardian, acknowledge that participation in Webb City Youth Cheer activities involves inherent risks, including but not limited to physical injury. I certify that my child is physically capable of participating in the program and that I assume all risks associated with their participation.
I hereby authorize Webb City Youth Cheer, its volunteers, coaches, and representatives to obtain medical treatment for my child in the event of an injury or medical emergency when I or the listed emergency contact cannot be reached. I understand that I am responsible for all medical expenses
incurred as a result of any injury sustained during participation in Webb City Youth Cheer activities.
I further release and hold harmless Webb City Youth Cheer, its board members, volunteers, and affiliates from any and all liability for injuries, damages, or medical expenses that may occur as a result of my child’s participation.
I understand that Webb City Youth Cheer does not provide medical insurance for participants. I
confirm that my child is covered under a personal or family medical insurance policy.
